At this time, we still have not received your signed
Form 1001. Because we have not
received these forms, your account has been restricted from
trading. As soon as we receive these forms, the
restriction will be removed. In the meantime, you may
access the account for trading by calling 401-642-6900.

You may print the Form 1001, or request that they
be mailed to you, from the Investor's Homepage under
Account Services, Apps and Forms. Please complete
these forms immediately and return them to SURETRADE. If
you neglect to submit your signed tax forms, you will be subject
to an IRS tax witholding of 31% of all sale proceeds in your
account. Please complete these forms to avoid this action.

If you provide your signed Form 1001 within
60 days of the date that you opened the account,
then SURETRADE will update the account appropriately.
If we receive your forms more than 60 days after the date
that you opened the account, and you have sold securities
in your account, then you must file with the Government
to obtain any tax witholdings that you may incur.
